APPOINTMENTS
- Appointments are available Monday through Thursday from 8:30 am to 4:15 pm. and Friday 7:30 a.m to 2:45 pm
- Kindly give 24 hours notice if canceling an appointment.
FEES
- Our Billing Department can discuss any fees in advance of services provided.
- Payments and/or co-payments are due at the time of service. We accept cash, checks, Visa, and MasterCard.
- Additional fees are rendered for a hearing test, allergy test, culture, nasal endoscopy, flexible laryngoscopy, and office surgery.
- If you have questions about a bill or want to make a payment, please contact the Billing Department.
INSURANCE BILLING POLICY
- We do not participate with every insurance plan. It is your responsibility to check with your plan before making an appointment. Our appointment scheduling team will ask about your plan when you call to make an appointment. We are required by the insurance carriers to collect your copay the day of your visit. Please note you are responsible for any deductibles, and non-covered services.
- If your insurance requires a referral, it is you responsibility to request it and have it sent to us prior to your visit. Your visit may need to be rescheduled if the referral is not received before your visit.
